The most common issue with TOTP security keys is a time desynchronization. If your phone's internal clock is even 15 to 30 seconds off from the SWTOR server clock, the generated codes will mismatch.

If you lose your phone and don't have your emergency code, go to the login page on SWTOR.com, leave the security key field blank, and click "Lost your Security Key?" to begin the automated email removal process.
